We require an on-site visit with you and your practice manager because we want to know you and your practice and we want you to know us. In addition to the onsite visit, the following information is necessary to determine your eligibility:

1. Recent (complete) Illinois State Credentialing Form
2. Report of procedures/patient encounters by CPT Code with RVUs if applicable
3. Confirmation of claims history with your current or past carriers
